City & Hackney Service - Social Prescribing

Family Action

Due to the current situation around Coronavirus (COVID-19), there will be no face-to-face sessions provided by the social prescribing service at Family Action for the foreseeable future. The welfare of our clients and staff is our highest priority and we are committed to continuing our support to those who need it. We have taken the decision to conduct all individual sessions and assessments over the telephone or online video until it is safe to return to normal working practices.


This service is open to anyone registered with a GP in City and Hackney over the age of 18.


Individuals will be offered an appointment with a Social Prescribing Coordinator who will conduct an in-depth needs assessment and then facilitate activate participation in local community based services, e.g. lunch clubs, gardening groups, benefits advice, exercise groups for the following groups of patients


  • Socially isolated
  • Frequent attenders to GP/A+E
  • Presenting with a social problem
  • Mild-moderate mental health problems
  • Keen to participate in non-clinical activities but not aware of what’s happening locally
  • People with any long term condition
  • Patients with a cancer diagnosis (they may be referred to the Macmillan Social Prescribing Scheme after initial assessment)
